If you believe facility data is incorrect or outdated, please include:
- The facility name and state
- Which data point appears incorrect
- The correct information (if known)
Note: Our data comes directly from CMS. If the information is correct in the CMS dataset, we cannot change it on our end. Facilities should contact CMS directly to update their official records.
Response time: We aim to respond within 48 hours on business days. Data correction reports are prioritized.
